The after-hours enquiry: where clinics quietly lose a third of their bookings
Look at your enquiry timestamps for a month. In most clinics we audit, between a quarter and a third of first-contact messages arrive outside opening hours, and the share is higher for aesthetics and for anything promoted on social media. Almost all of them get the same answer: nothing until morning.

When patients actually reach out
People research treatments on the sofa, not at their desk. The peaks are 8 to 11 pm on weekdays and most of Sunday. Working parents message during the school run. Shift workers message at 6 am.
None of these people expect you to be open. All of them expect a reply, because the last three businesses they messaged replied instantly.
What a missed enquiry costs
Take your average first-visit value, including the treatment plan that usually follows, and multiply it by the number of after-hours enquiries you did not convert. For a dental practice with a $600 average first visit and 40 after-hours enquiries a month, converting even a third of them is $8,000 a month.
The cost is invisible because it never appears in a report. Nobody logs the patient who booked elsewhere.
Why voicemail and auto-replies do not work
Voicemail asks the patient to do more work. 'We will reply in the morning' tells them to keep looking. Both are honest and both lose the booking, because the patient's problem is not information, it is a slot in a calendar.

Whatever you use, the test is the same: can it book the appointment, not just acknowledge the message?
- Reply within seconds on the channel the patient used (WhatsApp, chat, phone).
- Answer the three questions every enquiry has: price, availability, what happens next.
- Offer a real slot from the real calendar, with the right practitioner and duration.
- Confirm in writing and send pre-visit instructions.
- Hand over to a person for pain, complaints and anything clinical, with the conversation attached.
- Report every morning: how many came in, how many were booked, who needs a call.
The short version
You do not need a night shift. You need something that can book a slot at 10 pm and tell you about it at 8 am. Measure the after-hours share of your enquiries this month; it is usually the single largest growth lever a clinic has that costs nothing to fix.
